Looks good generally
After doing a quick seems really nice. The one thing worth adding that is comparatively easy (it would also be nice to have coordinates reportable in UTMs, but that is way too much work) is that Associated with quality there is an option for other, but then no free form text box directly with it to write out their description
Excellent documentation app!
This is exactly what is needed for emergency medicine personnel to document medical incidents. The app is easy to use, has a clear progression and helpful integrations such as location coordinate finding, photo uploading and sharing (email the note) functions. Great job to NOLS WMI for developing this! A go-to app for getting all the info down about a medical emergency in a clear format.

Im a WEMT through NOLS and work in the South as an EMT. I take groups out on treks through many of the state parks. This app is excellent! It allows for simple & easy input of pt. information, it autosaves so you wont lose the information, allows for GPS coordinates and the ability to disburse the information to whoever you handing the pt. off to. Great app!

Clean, easy, helpful! I am really really excited to have this app! And its free! I like the little "hint" questions for each box. I was especially excited to be able to include photos and email straight from the app. A couple additions for the next update: - add "itchy" to the quality list - include a first name line more than just initials - add an additional comments (I want to be able to add add comments after the accident has been resolved) - link with cloud (drop box, Google) for long term documentation)
